Received 14 December 2014; accepted 11 April 2015; published 20 April 2015ABSTRACTDepression is a common disorder amongst the general population and frequently encountered by most physicians. Selective Serotonin Re-Uptake Inhibitors (SSRI’s) have become the most commonly prescribed antidepressants due to their superiority compared with other antidepressants in the treatment of acute major depression. Although exceedingly rare, hepatotoxicity resulting from sertraline use has been previously reported. In these case reports, the liver injury pattern was predominately hepatocellular. Unlike previous cases, we report the case of a patient presenting with markedly elevated cholestatic enzymes and painless jaundice while taking sertraline for treatment of depression. Keywords: Sertraline, Obsessive-Compulsive Disorder, Acute Hepatitis1. Introduction Depression is a very common disorder amongst the general population and frequently encountered by most physicians. By Mike Mc Evoy Generic Name: Sertraline (multiple manufacturers) Common Brand Name: Zoloft (Pfizer Labs – U. S.) Popularity: 22nd most commonly prescribed drug between 2002 – 2007 (U. S.) Class: Antidepressant, SSRI (selective serotonin reuptake inhibitor)Treatment Uses – Treatment of major depressive disorder (MDD) in adults, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), premenstrual dysphoric disorder (PMDD), and social anxiety disorder. Sertraline has also been used effectively in treatment of aggressive behavior and dysthymia (a depressive disorder of less severity and longer duration than MDD). Sertraline has been helpful in treatment of alcoholism, Alzheimer’s disease, some pain syndromes, low blood pressure associated with hemodialysis, dyspnea associated with COPD, post-stroke emotional lability, and treatment of premature ejaculation. Investigational uses (under study) of sertraline include eating disorders, impulse control disorders, generalized anxiety disorder (GAD), and treatment of mild agitation associated with dementia in nonpsychotic patients. Sertraline has not been helpful in treating schizophrenia or eating disorders. Food and Drug Administration followed suit, but later admitted increasing skepticism about links between antidepressants and suicide risk in children and teens.

Many people with depression are prescribed antidepressant medications by a health care provider. Antidepressant medications not only affect biochemicals in the brain to help alleviate symptoms of depression, they may be impacting other organs in the body. The liver is one such organ that may be affected by antidepressant medications, according to the July 2007 issue of the “Annals of Pharmacology.” Antidepressants are medications used to treat major depression, dysthymia or chronic low-grade depression, and anxiety disorders such as obsessive compulsive disorder and social anxiety disorder. The type most people are familiar with is the selective serotonin reuptake inhibitors, or SSRIs, which includes the drugs Prozac, Zoloft and Celexa. Other classes of antidepressants are the tricyclic antidepressants; monoamine oxidase inhibitors, or MAOIs; serotonin-norepinephrine reuptake inhibitors, or SNRIs; and norepinephrine-dopamine reuptake inhibitors. Antidepressants may affect liver enzymes, as noted by the “Annals of Pharmacology” article. After starting antidepressants, the liver enzyme count in the bloodstream may rise. In many instances this rise is not concerning and the enzyme levels return to normal in a little over a week. Sertraline is a type of antidepressant known as a selective serotonin reuptake inhibitor (SSRI). It's often used to treat depression, and also sometimes panic attacks, obsessive compulsive disorder (OCD) and post-traumatic stress disorder (PTSD). Sertraline helps many people recover from depression, and has fewer unwanted side effects than older antidepressants. Sertraline comes as tablets, which are available only on prescription. Sertraline can be taken by adults for depression or obsessive compulsive disorder. Sertraline can be taken by children aged 6 to 17, but only for obsessive compulsive disorder. Check with your doctor before starting to take sertraline if you: If you have diabetes, sertraline can make it more difficult to keep your blood sugar stable. You can choose to take sertraline at any time, as long as you stick to the same time every day.
